For Beneficiaries

Medicare has started a program where doctors and other health care providers can more easily work together to coordinate services and improve patient care for Medicare fee-for-service (FFS) beneficiaries. This is known as an accountable care organization (ACO).

Signing up with your doctor and CVS Accountable Care does not limit or restrict access to your existing Medicare benefits, and you will always have the right to use any doctor, health care professional or hospital that accepts Medicare.

As a CVS Accountable Care beneficiary, you could benefit from extra services, at no additional cost to you. We work together with your regular team of Primary Care and Specialty doctors to provide support at home and over the phone as you need it.

Preventive care

  • Extra support and focus for your chronic conditions (such as diabetes, high blood pressure and heart failure)
  • Connecting you into Primary Care services, if you don't already have a doctor
  • Monitoring and coordinating preventive care and wellness visits
  • Medication review and education
  • On-site and virtual access to nutritionists, social work and other services

Care for an illness or unplanned event

  • Connecting your providers as you move between places of care (e.g., leaving the hospital or entering a skilled nursing facility)
  • Support after a hospital stay to help you recover at home
  • Access to care management for complex or serious illnesses

Improving care coordination

  • Coordinating your care needs through local and virtual resources including social worker support
  • Removing barriers to appointments and follow-ups through community-based services

How does CVS Accountable Care work with my doctor?

Your doctor and their staff may work behind the scenes with the ACO to coordinate your care, helping schedule appointments and making sure you get the follow-up care you need when you need it.

Other care team members may contact you to provide additional support and guidance, always in close collaboration with your regular doctors. The program does not limit or restrict access to your existing Medicare benefits.
