Confirming your doctor through voluntary alignment

Your doctor could be taking part in a new Medicare program, an Accountable Care Organization (ACO), with CVS Accountable Care.

Voluntary Alignment is a process that allows you to confirm your doctor or medical group is where you go for your regular care. This process will align you to the ACO with your doctor. By aligning, you would be eligible for additional services and support provided by the ACO, at no additional cost to you.

If you have traditional Medicare Part A and B benefits and are not already enrolled in another accountable care organization (ACO), you are eligible.

You can conveniently complete a form online through the CVS Accountable Care online portal.
